What to Expect After Botox: A Day-by-Day Guide

Day-by-day Botox aftercare and recovery — what to do, what to avoid, and how the results emerge over the first 14 days. From a Newmarket clinic with 10+ years of injecting.

If you've just had Botox or are about to, you've probably searched some version of "what to expect after Botox" — and you've probably gotten conflicting answers. The aftercare guidance you'll see online ranges from "do nothing different" to "lie flat for 4 hours, no exercise for a week." The reality is in between, and most of the rules exist for a specific reason.

Here's the day-by-day guide we walk every Botox client through at our Newmarket clinic. It applies whether you got Botox or Dysport — the protocol is essentially identical.

Immediately after (first 4 hours)

What you might notice right away:

  • Tiny pinpoint marks at each injection site (resolve within 30 minutes)
  • Mild redness or swelling at injection sites (resolves within an hour or two)
  • A slight headache (occasional; usually mild)
  • No movement change yet — Botox does not work instantly

Rules for the first 4 hours:

  1. Stay upright — no lying flat, no upside-down yoga, no head-down positions. The reasoning is to keep the Botox where we placed it before it binds to the neuromuscular junction.
  2. Don't rub or massage the treated areas — same reason. Rubbing can migrate Botox to nearby muscles you didn't want treated, which is how unwanted side effects (drooping eyelids or asymmetric brows) sometimes happen.
  3. No vigorous facial movements on purpose — but don't worry about normal facial expression; that's fine.

What you can do:

  • Walk, talk, eat, drink (just no strenuous exercise)
  • Apply gentle makeup if needed (light hand, no scrubbing)
  • Return to work or social plans

Day 1 (24 hours after)

What you might notice:

  • The injection-site marks should be gone or barely visible
  • You may feel a faint heaviness in the treated muscles — this is normal
  • No movement change yet — Botox is still binding

Rules for day 1:

  1. No vigorous exercise — moderate cardio is fine; heavy weightlifting, hot yoga, or anything that flushes blood significantly to the face should wait until tomorrow
  2. No facials, microdermabrasion, or aggressive skincare — let the skin settle
  3. Avoid alcohol — it increases the chance of bruising in the first 24 hours

Day 2 to 3

This is when many clients start to feel the first hint of effect — a slight tightness in the forehead or between the brows. The full effect is still 5 to 14 days away.

What's safe to resume:

  • Vigorous exercise (back to your normal routine)
  • Saunas, hot showers (avoid extreme heat the day of treatment, fine after that)
  • Facials and other skincare appointments — wait at least 7 days for treatments that involve pressure or heat
  • Alcohol in normal amounts

Day 5 to 7

Most clients see noticeable softening in the treated areas by day 5 to 7. The forehead lines you furrow when concentrating start to look less defined. The frown lines between the brows soften.

If you're not seeing change yet, don't worry — full effect can take up to 14 days. Resist the urge to compare with friends who say they "saw results in two days." That's typically the heaviness sensation, not the actual movement reduction.

This is also when you'll start to evaluate the result and form an opinion. If you think a specific area is under-treated, note it but don't decide yet — wait until day 14 for the full picture.

Day 10 to 14

This is the full-effect window. What you see on day 14 is what you have for the next 3 to 4 months. Specific things to evaluate:

  • Symmetry — do your forehead and brows move evenly? Slight asymmetry can sometimes happen and is easily corrected with a small touch-up.
  • Movement preservation — at our clinic, we deliberately preserve some natural movement on first-time clients (no one wants the "frozen" look). If you wanted more aggressive smoothing, this is the conversation to have at your touch-up.
  • Areas that didn't respond as expected — sometimes a small dose adjustment in a specific spot is needed.

If anything seems off, this is when you call your injector. Most quality clinics — including ours — offer a complimentary touch-up window in the first 14 to 21 days after treatment.

Day 30 onward

By day 30, the result is fully settled. Many clients describe feeling "their best" between weeks 2 and 8. The effect gradually softens between weeks 8 and 16, and most clients return for maintenance every 3 to 4 months.

Common questions in the recovery window

Will I look frozen?

Not if your injector is doing it right. Conservative dosing for first-time clients is the standard at quality practices. We deliberately under-treat slightly so we can refine on the second visit if needed. The goal is "you, on a good day" — not a different face.

What if I see drooping or asymmetry?

A slight asymmetry in week 2 can usually be corrected with a small dose to one side. A genuine drooping eyelid is rare but possible, especially in the first 7 to 14 days; it resolves on its own within weeks but call your injector to discuss. We can also prescribe eyedrops that help.

Can I get a facial after Botox?

Wait at least 7 to 14 days. Pressure and heat on freshly-treated areas can migrate the Botox.

Can I exercise normally after Botox?

After 24 hours, yes. Within the first 24 hours, keep it moderate.

How soon can I get more Botox?

The minimum interval is 12 weeks (3 months). Less than that and your body may start producing antibodies that reduce future effectiveness. We schedule maintenance appointments at 3 to 4 month intervals.

What if I'm getting filler too?

If both treatments happen the same day, Botox is typically done first. If on different days, we usually do Botox first and filler 7 to 14 days later — by then the Botox is settled and we can plan filler placement around the new resting muscle position.

When to call your injector

Most post-Botox concerns are normal and resolve. But call your injector immediately if you experience:

  • Difficulty swallowing or breathing — extremely rare but treat as urgent
  • Vision changes or persistent eyelid drooping that doesn't resolve in a few days
  • Significant fever, flu-like symptoms, or unusual swelling
  • Severe asymmetry that's clearly more than a touch-up

The 14-day rule for evaluation

If we leave you with one piece of advice from this guide: don't evaluate your result before day 14. The most common mistake clients make is panicking on day 5 ("I look the same!") or feeling oversmoothed on day 7 ("I feel weird!") and either calling for an emergency touch-up or deciding never to do it again. Wait the full two weeks. The result you have on day 14 is the real result.
